To help with formulating your meal I ask that you download the forms linked below and return them as soon as you can. The meal planning process won't go forward without these forms, please fill them out and email them to evolvent.pdx@gmail.com.
We'll communicate via email or phone to dial in your food preferences and make sure your needs are going to be met and you feel supported.
The meal plans will be delivered in a PDF form every two weeks, and we can make up to four adjustments every two week period.
